Patient Information

Patients are seen by Leonard Gordon by appointment to permit the most efficient care possible with a minimum amount of waiting. When you call for your appointment, please give the receptionist a brief idea of your medical problem and your insurance information.

Please bring the following items with you:

  • Photo ID
  • Your current valid insurance card
  • Referral authorization from your primary care provider if necessary
  • List of all medications, including dosage taken currently
  • All previous x-rays, MRI scans, or other test results relating to your medical problem
  • Previous medical records relating to your medical problem

Contact our receptionist during office hours, or schedule a follow-up appointment as soon as you leave our office. If you must cancel an appointment, please call at least 24 hours in advance, so that we may accommodate another patient.

We know that your time is important, and strive to see patients at their scheduled appointment time, but due to the complexity and unpredictability of our patient's problems, unavoidable delays may occur. We appreciate your patience and understanding at these times.


Leonard Gordon accepts most major insurance carriers. For more information or to determine what your health plan covers, please call our office. We are happy to verify your plans specific benefits.

When you come for your first visit, please have your insurance information available. If your insurance requires that you have a referral form from your primary care physician, you must present it at your appointment. If you do not have the required items, your evaluation may be delayed, and you may be asked to reschedule your appointment.

Fees & Payments

Our fees vary, depending upon the service provided. Charges for x-rays and other services are in addition to the physician fees.

We require that patients pay for the visit and treatment at the time of service. Our business office accepts cash, check, Visa, Mastercard and American Express. If you anticipate difficulty making your payments, please contact our billing department to discuss a payment plan.

Workers' Compensation

If you have worker's compensation coverage, the proper authorizations are obtained before your appointment. Our office can help expedite the process. You may be required to complete additional forms when you arrive at your appointment.

If you need an interpreter, please inform us so that one can be scheduled.
